Overview

The Continued Health Care Benefit Program (CHCBP) is a premium-based plan that bridges the gap when you lose eligibility for regular TRICARE — typically after separation from active duty, loss of dependent status, or aging out of TRICARE Young Adult.

Up to 36 months

You can enroll for up to 36 months. Certain unremarried former spouses and children may qualify for longer coverage under specific rules.

2025 premiums

2025 quarterly premiums: $1,654 individual and $4,079 family. Premiums are paid quarterly in advance. In addition to premiums, you pay TRICARE Select cost-shares and the annual deductible for the care you receive.

How to enroll

How it works: CHCBP uses TRICARE Select rules — see any TRICARE-authorized provider, no PCM, no referrals for most care. The program is administered by Humana Military for the entire country (not split by region). Apply within 60 days of losing TRICARE coverage by submitting DD Form 2837 and your first quarterly premium.

When to use it

CHCBP is not a long-term solution. Use it as a bridge to employer coverage, the ACA Marketplace, or eligibility for TRICARE For Life at Medicare age.
