## Quick answer When living or traveling overseas, your easiest way to get prescriptions is through a military hospital or clinic pharmacy at $0 cost. If you use a local civilian pharmacy, you must pay the full price upfront and file a claim for reimbursement, which is subject to your plan's deductible and cost-shares.

Managing prescriptions outside the United States involves different rules than the domestic "retail pharmacy network" we use stateside. There are three primary ways to get your medicine while overseas.

### 1. Military Treatment Facility (MTF) Pharmacies The most cost-effective option is using a pharmacy located inside a military hospital or clinic. * **Cost (2026):** $0 for all beneficiaries. * **Availability:** Check the local facility's formulary, as overseas bases may have limited stock of niche medications.

### 2. TRICARE Pharmacy Home Delivery (Express Scripts) You can use mail-order through Express Scripts in most overseas locations, but there are strict limitations. * **Where it works:** APO/FPO addresses and certain U.S. Embassies. * **Where it doesn't work:** You cannot use Home Delivery if you have a local civilian address in a country that prohibits the importation of prescription drugs (e.g., Germany, Japan, and the Philippines have significant restrictions). * **Cost (2026):** Generally the same as domestic rates (e.g., $16 for a 90-day supply of generics; $43–$48 for brand name).

### 3. Overseas Host Nation (Civilian) Pharmacies If you are not near a base and cannot use mail-order, you must use a local civilian pharmacy. * **The Process:** Unlike the U.S., there is no "network" that bills TRICARE directly. You pay 100% of the cost at the counter in local currency. * **Reimbursement:** You must file a manual claim with **International SOS** (the overseas contractor). * **Costs (2026):** Your reimbursement will be the drug cost minus your plan's applicable deductible and cost-shares. For TRICARE Select beneficiaries, this often means paying 20–25% of the cost after the deductible is met.

### Special Rules for the Philippines In the Philippines, you must use a **Certified Pharmacy**. If you use a non-certified pharmacy, TRICARE will deny your claim. International SOS maintains a list of these specific locations to prevent fraud and ensure drug quality.

### Filling Overseas Prescriptions A prescription from an overseas civilian provider can usually be filled at a host-nation pharmacy. However, to fill a prescription at an MTF or via Express Scripts, it usually must be written by a U.S.-licensed or "authorized" provider. If you see a local doctor, you may need to take that prescription to the base clinic to have a military provider "translate" or rewrite it for the MTF pharmacy.

## Who this affects * **Active Duty Service Members:** Generally $0 using any pharmacy, but must have oversight by the Primary Care Manager. * **Active Duty Family Members:** On TRICARE Overseas Program (TOP) Prime or Select. * **Retirees and their families:** Living abroad (TOP Select). * **TRICARE For Life beneficiaries:** When traveling or living overseas. * **Guard/Reserve members:** On TAMP or TRICARE Reserve Select while overseas.
