## Definition A Primary Care Manager (PCM) is a healthcare provider—usually a doctor, nurse practitioner, or physician assistant—who serves as your first point of contact for medical care and acts as the "gatekeeper" for specialist referrals under TRICARE Prime plans.

## What it means in practice If you are enrolled in a TRICARE Prime plan (including TRICARE Prime Remote or the US Family Health Plan), you are required to have a designated PCM. This provider is responsible for your routine care, such as annual wellness exams, immunizations, and treatment for non-emergency illnesses. Most importantly, your PCM is responsible for coordinating your overall healthcare strategy; if you need to see a cardiologist, dermatologist, or any other specialist, your PCM must submit a referral request to your regional contractor (Humana Military in the East or TriWest in the West) for approval.

For active duty service members (ADSMs), the PCM is typically assigned at a Military Medical Treatment Facility (MTF). Active duty family members and retirees may be assigned an MTF provider if space is available, or they may select a civilian provider within the TRICARE network. If you seek specialty care without a referral from your PCM, you may be subject to "Point-of-Service" charges. For the 2026 plan year, these charges typically include a $300 deductible for individuals ($600 for families) and a 50% cost-share of the TRICARE-allowable amount for the visit.

While TRICARE Select beneficiaries do not have a mandatory PCM, they are still encouraged to establish a relationship with a primary doctor. However, Select users have the freedom to visit any TRICARE-authorized provider without a referral, whereas Prime users must stay within the PCM structure to keep out-of-pocket costs at their lowest—often $0 for active duty families. You can change your PCM at any time by logging into the Beneficiary Web Enrollment (BWE) portal or calling your regional contractor.
