## Definition Palliative care overseas is specialized medical care focused on providing relief from the symptoms and stress of a serious illness for TRICARE beneficiaries living outside the United States.

## What it means in practice If you are living overseas—whether as an active duty service member, a retiree, or a family member—palliative care is available to improve your quality of life during a serious or chronic illness (such as cancer, heart failure, or advanced respiratory disease). Unlike hospice care, which is specifically for patients nearing the end of life, palliative care can be provided at any stage of an illness and alongside curative treatments.

In the TRICARE Overseas Program (TOP), palliative care services are managed by the overseas regional contractor, **International SOS**. Because healthcare standards and legal definitions vary significantly by country, the "palliative" services you receive in Germany or Japan may look different than those in the United States. Coverage typically includes specialist consultations, pain management, and emotional support. However, because TRICARE generally follows Medicare guidelines, these services must be deemed "medically necessary" and ordered by a physician.

For 2026, costs depend on your plan (TOP Prime or TOP Select) and your beneficiary status. Active duty service members and their families on TOP Prime generally have $0 out-of-pocket costs for authorized care. Retirees on TOP Select may be subject to a deductible and cost-shares (often 20% to 25% of the TRICARE-allowable charge). It is critical to coordinate this care through your Primary Care Manager (PCM) and International SOS to ensure your providers are "TRICARE-authorized." If you use a non-authorized provider without a waiver, you may be responsible for the entire bill.

In remote locations or developing nations where palliative specialists are unavailable, TRICARE and International SOS may coordinate "medical evacuation" (MedEvac) or "cashless/claimless" access to the nearest Center of Excellence. Always contact the TOP Regional Call Center for your specific area (Eurasia-Africa, Latin America-Canada, or Pacific) before starting palliative treatments to ensure authorization is on file.
