## Definition Palliative care overseas is specialized medical care for TRICARE beneficiaries living outside the United States that focuses on providing relief from the symptoms and stress of a serious illness, regardless of the patient's prognosis.

## What it means in practice While often confused with hospice, **palliative care** under TRICARE is not limited to end-of-life care. If you are stationed or living abroad in the East, West, or Latin America/Canada regions, you can receive palliative care at any stage of a serious illness (such as cancer, heart failure, or ALS). The goal is to improve quality of life for both the patient and the family by managing pain, spiritual distress, and complex symptom mapping while the patient continues to receive curative treatments.

To access these services overseas, beneficiaries must work through **International SOS**, the TRICARE Overseas Program (TOP) contractor. For TRICARE Prime Overseas and TRICARE Prime Remote Overseas members, a referral from a Primary Care Manager (PCM) and prior authorization from International SOS are typically required. Without this authorization, care may be processed under the Point-of-Service (POS) option, resulting in significantly higher out-of-pocket costs (a $300 individual deductible and 50% cost-shares for 2026).

The availability of palliative care depends heavily on your location. In highly developed medical systems like Germany or Japan, "palliative support" is well-established. In more remote "remote" locations, TRICARE may need to coordinate with host-nation providers who may have different standards of practice than those in the U.S. Coverage generally includes consultations with specialists, pain management medications through **Express Scripts**, and social work support.

Financially, for Active Duty Service Members (ADSMs), there are $0 out-of-pocket costs for authorized palliative care. For Active Duty Family Members (ADFMs) on TRICARE Select Overseas, costs will include an annual outpatient deductible (varies by rank and plan year) and a percentage of the allowable charge. Because overseas providers are often "non-participating," you may be required to pay the provider upfront in full and file a claim for reimbursement with International SOS.
